Please see the list below of all the items required to include your company on our accredited vendor’s list.

(Note: #2 & #3 only apply if required by law)

 

  1. Current Business Licenses for any/all cities you will be providing services for
  2. Contractors License (if required)
  3. Current special permits required in your line of work
  4. State of Nevada Department of Taxation Certificate
  5. Current Workman’s Compensation Insurance Certificate, reflecting Excellence Community Management LLC as additionally insured
  6. Current Liability Insurance Certificate, reflecting Excellence Community Management LLC as additionally insured
  7. Completed and signed W-9 for reporting your earnings (Federal Law requires we obtain a federal identification number)
  8. Telephone, Facsimile, Cell, Email Address, Mailing Address, Contact Name

Please note all bids are requested by Property Managers, not the Accounting Department. If you have already been contacted by one of our Manager’s, please submit the proposal directly to that person.
